About the position

The Clinical Case Manager Behavioral Health is responsible for care coordination and case management for members up to age 20 who have experienced a crisis event. This role involves evaluating physical and behavioral health needs, facilitating overall wellness, and ensuring compliance with clinical coverage policies. The position requires significant travel within Illinois to meet members face-to-face, with a focus on enhancing member wellness through integrated care strategies.

Responsibilities

• Execute evaluation of member needs and benefit plan eligibility.
• Facilitate member transition to the organization's programs and plans.
• Apply advanced clinical judgment to reduce risk factors and address complex health indicators.
• Handle reviews of prior claims to assess impact on current case management.
• Create a holistic approach for referrals to clinical resources and assess functionality.
• Ensure case management processes comply with organization and regulatory requirements.
• Implement systems to maximize member engagement and assess health status.
• Coach and train junior colleagues in techniques and responsibilities.

Requirements

• Active, unrestricted LCSW, LCPC, or LMFT in the state of Illinois.
• Minimum two years experience working with children.
• Willing and able to travel up to 50-75% of their time to meet members face to face.
• Reliable transportation required; mileage is reimbursed.
