About the position

The position involves utilizing advanced clinical judgment and critical thinking skills to facilitate appropriate member physical health and behavioral healthcare. This includes assessment and care planning, direct provider coordination, and coordination of psychosocial wraparound services to promote effective utilization of available resources and optimal outcomes. The role is fully remote, operates Monday to Friday during standard business hours, and does not require weekend or holiday work.

Responsibilities

• Conduct comprehensive assessments of referred member's needs and eligibility using clinical tools and data review.
• Determine approach to case resolution and meeting member needs by evaluating benefit plans and available programs/services.
• Apply clinical judgment to incorporate strategies designed to reduce risk factors and address complex clinical indicators impacting care planning.
• Perform crisis intervention with members experiencing behavioral health or medical crises and refer them to appropriate clinical providers.
• Provide crisis follow-up to ensure members receive appropriate treatment/services.

Requirements

• 3 years of direct clinical practice experience post master's degree in a hospital or alternative care setting.
• Knowledge of mental health and substance abuse disorders.
• Experience using Motivational Interviewing and good technological skills.
• Valid unrestricted independent professional behavioral health clinical license to practice in the state of residence.
