About the position

The Clinical Case Manager Behavioral Health position at CVS Health is a full-time remote role focused on providing telephonic clinical case management for the Medicaid population, particularly those with histories of trauma or in foster care. Responsibilities

• Conduct comprehensive assessments of referred members' needs and eligibility using clinical tools and data review.
• Perform crisis intervention for members experiencing behavioral health or medical crises and refer them to appropriate providers.
• Develop and monitor care plans in collaboration with members and their care teams to meet established goals.
• Utilize case management processes in compliance with regulatory guidelines and company policies.
• Engage in telephonic outreach and documentation in a clinical case management platform.
• Consult with supervisors and medical directors to overcome barriers to care and present cases at conferences for multidisciplinary input.
• Analyze utilization and clinical data to identify comprehensive member needs.

Requirements

• Must reside in specified counties in Michigan.
• Active unrestricted LMSW or LPC in the state of Michigan.
• 1+ year of case/care management experience.
• 4+ years of direct clinical experience post-master's degree in relevant settings.

Nice-to-haves

• Experience working with the foster care population and/or children with histories of trauma.
• Experience with complex disease management across age groups.
• Proficiency in Microsoft Office products including Word, Excel, Teams, and Outlook.
