Responsibilities

  • Responsible for a wide range of clinical functions, including review of appropriate drug therapy.
  • Calculates in evaluating serum drug concentrations in relation to pharmaceuticals, dose and inter-patient variables in select pharmacotherapy patients.
  • Actively participates in outstanding customer service and maintains respectful relationships.
  • Coordinates and provides clinical pharmacy services for patients receiving treatment under the direction of the Preceptor.
  • Monitors for appropriateness of medication, dose, strength, expiration date, route of administration and dosage form.
  • Documents clinical interventions to prevent or eliminate drug-related problems.
  • Prepares and submits concise activity reports documenting interventions and potential changes in patient outcomes.
  • Provides specialty consults as assigned in a timely, accurate and consistent manner.
  • Calculates pharmacokinetic parameters and communicates recommendations.
  • Educates and counsels patients on medication usage and possible side effects.
  • Participates in education presentations to healthcare personnel on pharmacokinetics and pharmacotherapy.
  • Coordinates and participates actively in staff development programs.

Requirements

  • Graduate of a Doctor of Pharmacy degree program.
  • Good communication skills and ability to work with staff members.

Nice-to-haves

  • Current valid State of Florida Pharmacist license.
  • Ability to work closely with medical, nursing and pharmacy services in pharmacotherapy.
  • Ability to work with pharmacy and medical students when appropriate.
  • Ability to take active part in pharmacy staff development.
  • Ability to serve the needs of neonatal, pediatric, adult, adolescent and geriatric patients.
