About the position

The position encompasses care of patients from intake through checkout. Provides assistance to the physician or advanced practice provider, telephone triage, chart organization, care of equipment, and care of the environment. Provides professional nursing skills in an outpatient clinic setting, emphasizing disease prevention and health maintenance. Works closely with the clinical staff, front office staff and other related departments to support the patient and family needs. Monitors and facilitates patient assistance programs such as Family PACT, CHDP, and all prescription assistance programs.

Responsibilities

  • Collects patient health data.
  • Analyzes the assessment data in determining diagnoses/patient problems.
  • Identifies expected outcomes individualized to the patient.
  • Develops a plan of care that prescribes interventions to attain expected outcomes.
  • Implements the intervention identified in the plan of care.

Requirements

  • Current California RN license
  • Current CPR card.
  • Demonstrates a willingness to learn, accept change, and assume different duties as needed.
  • Knowledge of diagnostic and procedural coding, insurance prior authorizations, and patient assistance programs.
