About The Position

The role of the Clinical Educator is to activate and support members with chronic kidney disease (CKD) by helping them understand their condition and manage it effectively. The role primarily focuses on educating members, their families, and caregivers about CKD, treatment options, and lifestyle adjustments to prevent progression and complications. The Chronic Kidney Disease Educator provides education and support to members diagnosed with chronic kidney disease, guiding them on how to manage their condition, improve quality of life, and slow the progression of the disease. The CKD Educator works closely with members, families, and internal and external healthcare teams to develop individualized care plans, offer nutritional guidance, discuss treatment options, and promote healthy lifestyle changes.

Requirements

  • High School Diploma or equivalent GED required.
  • 3+ years of nephrology and/or healthcare/managed care experience required.
  • Knowledge of computers, faxes, printers and all other equipment.
  • Proficient in MS Office programs (i.e., Word, Excel, Outlook, Access and Power Point).
  • Ability to deal with responsibility with confidential matters.
  • Ability to work in a multi-task, high stress environment.
  • Ability to prioritize and meet deadlines.
  • Attend meetings and trainings as required.

Responsibilities

  • Educate members about CKD, its stages, symptoms, and progression.
  • Teach members about the importance of kidney function and how to monitor their health (e.g., blood pressure, weight, lab tests).
  • Provide guidance on managing comorbid conditions such as diabetes, hypertension, and cardiovascular disease, which affect kidney health.
  • Offer information about treatment options, including dialysis and kidney transplantation, if applicable.
  • Assist in the development of personalized dietary plans with an emphasis on low-sodium, low-protein, and kidney-friendly foods.
  • Counsel members on managing fluid intake, proper hydration, and controlling potassium, phosphorus, and other electrolytes.
  • Encourage and support physical activity and other healthy lifestyle modifications.
  • Act as a resource for members, addressing concerns and answering questions about managing CKD.
  • Provide emotional support to members and families, especially for those dealing with advanced stages of CKD or requiring dialysis.
  • Work with multidisciplinary teams to address member care needs and improve treatment adherence.
  • Collaborate with nephrologists, nurses, dietitians, and other healthcare providers to create comprehensive care plans for members.
  • Ensure communication between healthcare providers, assisting in coordinating care, referrals, and follow-up appointments.
  • Monitor member progress and educate on the importance of ongoing monitoring (e.g., lab tests, kidney function).
  • Promote preventive measures to reduce the risk of CKD progression, such as smoking cessation, weight management, and controlling blood sugar and cholesterol levels.
  • Provide information on renal replacement therapy options (dialysis, transplant), and help members make informed decisions about their care.
  • Maintain accurate records of member education and progress in compliance with healthcare regulations.
  • Track member outcomes and adjust educational plans as necessary to meet member needs.
  • Submit reports and contribute to quality improvement efforts related to CKD care.
  • Complete assigned tasks accurately and within specified time limits.
  • Maintain smooth working relations with co-workers and supervisors. Be a “Team Player”.
  • Anticipate future events, set realistic goals, timetables, coordinate activities to ensure smooth work flow and time management.
  • Schedule ongoing training sessions for CKD education.
  • When necessary, assist in the creation of new training materials based on current CKD guidelines.
  • Verify that member confidentiality is maintained and that HIPPAA compliance is observed at all times.
  • Other duties as assigned by management
