About the position
The Chronic Care Health Educator will collaborate with all care team members to ensure that patients are getting the care they need to meet their care goals and manage their chronic conditions. The Chronic Care Health Educator will communicate directly with the patient in person or by phone. They will provide education, care coordination follow ups and appointment assistance. The Chronic care health educator works closely with the medical team to coordinate the care plan needs and follow ups.
Requirements
- High School diploma
- Tuberculin Skin Test (TST)
- Hep B Vaccine or Declination
Nice-to-haves
- Associate degree in health education or related field
- 6 months of clinical experience in a clinical environment
