About The Position

Bristol Hospice is seeking an experienced Hospice Chaplain to join their team in the North West Territory from Bangor. This full-time position involves meeting the spiritual needs of patients and families through assessments, care plans, and collaboration with the Interdisciplinary Team. Bristol Hospice is a nationwide leader focused on a family-centered approach to hospice care, dedicated to treating patients and families with compassion, respect, and dignity.

Requirements

  • Graduate of an accredited seminary or school of theology, BD, M.Div. or equivalent theological degree preferred.
  • Must have experience working with death and dying, individuals/family/caregiver.
  • Hospice experience preferred.
  • Working knowledge of Microsoft Word, Excel, PowerPoint, and EMR systems.
  • Must be willing to travel to hospice locations as assigned and be flexible with working hours.

Responsibilities

  • Assure spiritual assessment of patients and families/caregivers in the hospice program and appropriate services are rendered in a timely manner.
  • Facilitate the development of the individualized plan of care by participating in the comprehensive assessment to meet identified spiritual needs.
  • Provide direct spiritual care to patients and families/caregivers.
  • Serve as liaison and support to community chaplains and spiritual counselors.
  • Maintain records of spiritual care services utilization and related activities for quality assurance, program development, and policies and procedures review and revision.
  • Document direct services and ongoing communication with community chaplains and spiritual counselors.
  • Attend patient care conference and interdisciplinary group meetings as a member of the interdisciplinary group.
  • Provide consultation, education and support to interdisciplinary group on spiritual care.
  • Recruit community chaplains, spiritual counselors and spiritual care volunteers adequate to meet patient and family/caregiver needs by developing community contacts and offering education through congregations about hospice care.
  • Provide for funeral or memorial services for patients as requested.
  • Plan periodic memorial services to meet the needs of personnel, volunteers and community clergy/spiritual counselors working with the hospice team.
  • Assist in supervision of spiritual care volunteers when assigned to patients/families/caregivers.
  • Actively participate in quality assessment performance improvement teams and activities.
  • Accept and perform other related duties and responsibilities, as required.
