About the position

Heartland Home Health is seeking a Certified Nursing Aide (CNA) for our Midland office. The CNA will report to the Director of Professional Services and will receive direction from the Patient Care Coordinator or Registered Nurse to carry out assigned duties that promote patient comfort and support. This role is essential in providing physical, spiritual, and emotional support to patients and their families, ensuring they can make the most of every moment.

Responsibilities

  • Provide personal care and support to patients in their homes.
  • Assist with daily living activities such as bathing, dressing, and grooming.
  • Take and record vital signs including temperature, pulse, respiration, and blood pressure.
  • Observe and report changes in the patient's condition to the nursing staff.
  • Develop and maintain rapport with patients and their families.
  • Work collaboratively with an interdisciplinary team to ensure comprehensive patient care.

Requirements

  • High School diploma or G.E.D.
  • Valid CNA License in the State per State and Federal Regulations.
  • Minimum of one year experience as a Home Health Aide, or currently enrolled nursing student with at least 6 months clinical experience, or completion of a Home Health Aide training program or certified nursing assistant course.
  • Ability to accurately take and record vital signs.
  • Ability to observe, report, and document changes in the patient.
  • Excellent communication skills, both verbal and written.
  • Ability to travel 100% on a daily basis.
