About the position

The Centralized Intake Specialist (CIS) plays a vital role in supporting individuals by collecting registration information, conducting initial screenings for eligibility, and verifying health insurance details to connect them to FSA's programs and services. This position is dedicated to helping individuals overcome barriers, find mental health resources, and navigate the system to access the care and support they need. The CIS will manage accurate data and maintain real-time communication with FSA staff to ensure efficient service delivery, all while working in a supportive, team-oriented environment.

Responsibilities

  • Conduct trauma-informed intake, registration, screenings, and triage to FSA's programs and services.
  • Create a welcoming initial client engagement experience, including program introduction and service explanations.
  • Understand and apply screening tools to make appropriate internal connections to FSA's programs and services.
  • Maintain current knowledge about health insurance, including Medi-Cal and other providers.
  • Notify Intake Program Manager of incoming referrals needing urgent response.
  • Facilitate access to crisis intervention by connecting individuals with appropriate resources.
  • Provide personalized support and guidance regarding mental health resources and treatment options.
  • Create individualized care plans by collaborating with individuals to identify appropriate interventions and referrals.
  • Continuously monitor and reassess individualized care plans to ensure they align with individuals' changing needs.
  • Advocate for individuals' needs and assist with transitions between different mental health service providers.
  • Ensure response to all requests for services within 24 business hours according to program protocols.
  • Enter required intake service information into FSA's client database within 24-48 hours of contact.
  • Participate in community events to promote FSA services and build connections with community members.
  • Attend professional development training courses and other required events.

Requirements

  • Strong understanding of mental health resources and services.
  • Experience in conducting intake and assessments in a healthcare or social services setting.
  • Ability to maintain confidentiality and handle sensitive information appropriately.
  • Excellent communication and interpersonal skills to engage with clients and community partners.
  • Proficiency in using databases and maintaining accurate records.
