About The Position

Performs complex (journey-level) casework in the provision of Supported Employment and Supported Housing services for individuals receiving services through Central Counties Services. Responsibilities include assisting individuals in obtaining and maintaining competitive integrated employment and stable housing through person-centered, recovery-oriented, trauma-informed, and evidence-based practices. Serves as a liaison between consumers, families, employers, landlords, community partners, and Center staff to facilitate access to employment and housing opportunities. Provides consultation and education to Center staff regarding Supported Employment and Supported Housing services throughout the Center’s catchment area. Works under the supervision of the assigned supervisor.

Requirements

  • Bachelor’s degree from an accredited college or university in a behavioral health, social services, rehabilitation, or related human services field required.
  • Applicants with degrees outside behavioral health or human services disciplines must provide documentation of qualifying coursework
  • Knowledge of mental illness, co-occurring disorders, recovery principles, rehabilitation methods, casework principles, and community resources.
  • Knowledge of Supported Employment, Supported Housing, and service coordination practices for individuals with behavioral health needs.
  • Ability to assess consumer needs, develop service plans, coordinate services, and monitor progress toward goals.
  • Ability to utilize Microsoft Office applications, electronic health record systems, and other technology to maintain records, track outcomes, and prepare reports.
  • Ability to effectively manage challenging situations and respond appropriately to escalated behavior in accordance with training and Center policies.
  • Demonstrates effective communication, consultation, critical thinking, attention to detail, time management, and problem-solving skills.
  • Exhibits sound judgment, professionalism, ethical conduct, confidentiality, and accountability in all work activities.
  • Ability to gather and analyze information, manage multiple priorities, and work independently with minimal supervision.
  • Establishes and maintains productive working relationships with consumers, families, coworkers, community partners, and the public while contributing positively to a collaborative team environment.
  • Must possess and maintain a valid Texas driver's license, an acceptable driving record, and reliable transportation to travel throughout the Center's catchment area on a daily basis.
  • Regular attendance, dependability, and reliability are essential functions of this position.

Responsibilities

  • Provides Supported Employment services including vocational assessment, job search assistance, resume development, interview preparation, benefits education coordination, job placement, job coaching, and employment retention services.
  • Develops and maintains relationships with local employers through ongoing employer outreach and engagement activities to identify competitive integrated employment opportunities for consumers.
  • Develops and implements evidence-based practices consistent with the IPS Supported Employment model and Center requirements.
  • Provides individualized support throughout the hiring, onboarding, and employment process, including follow-along services designed to promote successful job placement, retention, and career advancement.
  • Participates as an active member of multidisciplinary treatment teams and provides employment-focused consultation consistent with IPS fidelity standards.
  • Cross-trains in both Supported Employment and Supported Housing services and provides services in both program areas as assigned.
  • Conducts housing assessments for eligible individuals and families and provides services based on assessment findings.
  • Assists consumers in identifying, obtaining, and maintaining safe, affordable, and stable housing.
  • Develops and maintains a current inventory of housing resources and available housing opportunities throughout the service area.
  • Coordinates with landlords, housing providers, community organizations, and other stakeholders to support housing stability and successful tenancy.
  • Meets productivity, service delivery, documentation, and program requirements by completing assessments, person-centered recovery plans, progress notes, and other required records in accordance with Center policy, HIPAA requirements, Texas Administrative Code, funding requirements, and applicable regulations.
  • Provides education and consultation to Center staff regarding Supported Employment and Supported Housing initiatives and participates in team meetings as required.
  • Maintains required certifications and competencies through completion of required training, continuing education, and professional development activities in accordance with Center and program requirements.
