About the position

The Case Manager at Sasha Bruce Youthwork plays a crucial role in supporting homeless young adults aged 18-24 through the Transitional Living Program. This program is designed to assist these individuals in acquiring the necessary skills and resources to live independently over a 24-month period. The Case Manager will work closely with youth and their families, utilizing their expertise to ensure that program requirements are met and that clients receive the support they need to thrive. The position requires a proactive approach to leadership, particularly in the absence of the Program Manager, and involves a commitment to empowering youth and families through various services including life skills training, employment assistance, and counseling. In this role, the Case Manager will oversee client assessments and case management services, ensuring that individual treatment plans are developed and implemented effectively. They will also be responsible for maintaining accurate documentation of all services provided, including client progress reports and any issues that require immediate attention. The Case Manager will assist clients with discharge and aftercare planning, ensuring a smooth transition to their next steps after completing the program. Familiarity with local services and resources is essential, as the Case Manager will need to assess each client's needs and facilitate appropriate referrals. The position also involves active participation in weekly staff meetings, live supervision with clients and their families, and ongoing training and professional development. The Case Manager will ensure that client files and records are kept secure and up-to-date, contributing to the overall effectiveness of the program and the well-being of the clients served.

Responsibilities

  • Demonstrates leadership and support for staff, especially in the absence of the Program Manager.
  • Supports program structure and all youth within the program.
  • Empowers youth and families through initiative and support.
  • Supervises and ensures house/office coverage, personal safety of clients, and enforces house rules.
  • Oversees client assessments and provides case management services.
  • Develops individual treatment plans for clients and families.
  • Provides timely and accurate documentation of services, including client progress reports.
  • Assists clients with discharge and aftercare planning.
  • Maintains familiarity with local services and assesses client needs for referrals.
  • Engages in weekly staff meetings and meets regularly with the Program Manager for supervision.
  • Participates in live supervision with clients and their families.
  • Participates in all training and in-services as identified by the Program Manager or Clinical Director.
  • Ensures client files and records are accurate, updated, and secure.
  • Performs other duties as assigned.

Requirements

  • Bachelor's Degree required; Master's Degree preferred or currently pursuing advanced education in Social Work, Case Management, Counseling, or a relevant Human Services field.
  • 5 years' experience in the human service field, preferably with adolescents and/or young adults.
  • Expert knowledge of issues surrounding homelessness.
  • Ability to work with community leaders to raise funding and lead complex projects.
  • Ability to partner effectively with individuals and families in difficult situations.
  • Strong familiarity with local DC community resources and services for young people and families.
  • Strong sense of personal responsibility, flexibility, and effective communication skills with diverse populations.
