About the position

The Case Manager is responsible for managing service plans and assessments for youth in a residential facility. This role involves conducting intake interviews, coordinating with legal representatives, and ensuring compliance with state and federal requirements. The Case Manager must maintain organized documentation and provide updates to families and stakeholders while being flexible with work hours, including weekends and on-call duties.

Responsibilities

  • Conduct initial intake interviews of youth and gather necessary information.
  • Interview family members and sponsors to verify information and relationships.
  • Determine options for youth and document the reunification process.
  • Coordinate with local attorneys for legal presentations and ensure youth receive necessary documentation.
  • Complete assessments and service plans in compliance with regulations.
  • Document all actions and communications with youth and stakeholders.
  • Submit reunification packets for review and ensure timely release of youth.
  • Provide regular updates to youth and their families via face-to-face and telephonic communication.
  • Facilitate communication between minors and their families or caregivers.
  • Coordinate weekly treatment team meetings and attend case staffing with funding representatives.
  • Maintain both physical and web-based client files.
  • Transport clients as needed for program services and appointments.

Requirements

  • Bachelor's degree in Social Work, Psychology, Human Services, Counseling, or related field.
  • 1 year of experience working with adolescents or in youth services (including volunteer/internship experience).
  • Proficient in Microsoft Windows applications (Word, Excel).
  • Ability to work a flexible schedule.
  • Cleared Tuberculosis test results and background check.
  • Bilingual in English and Spanish is required.
  • Valid Driver's License.

  • Multilingual skills in Turkish, Arabic, Punjabi, Hindi, Bengali, or Portuguese.
