About the position

New Growth Recovery is seeking a Case Manager for their outpatient substance use program in Springfield, MA. The program provides Day Treatment and Outpatient services (PHP/ IOP/ OP) services to adults (18+). Are you driven, ambitious, and solution-focused? The Case Manager is responsible for performing a wide variety of client care activities, as it relates to case management services, treatment planning, continuing care planning, and discharge planning. The Case Manager will collaborate with Treatment Team members to formulate individualized treatment plans and assist in coordinating discharge planning for each patient. The Case Manager will also have the responsibility for maintaining communication with the referral source and other services provided.

Responsibilities

  • Provides case management sessions to clients.
  • Document the client’s progress accurately in the Electronic Health Record (EHR) system, and in a timely manner.
  • Present psychosocial and diagnostic information to treatment team in a clear, concise manner.
  • Review initial treatment plan and any case management issues during first session.
  • Continue treatment planning process throughout patient’s treatment stay (i.e., medical, legal, and employment issues, problematic behaviors, etc.) and coordinate with treatment team to complete plan.
  • Assist client with internal and external resources (i.e., community agencies, local support groups, etc.) as needed on continuing care plan.
  • Assist client in managing outside stressors (legal, financial, vocational, etc.) according to the best interests of the client’s progress in recovery.
  • If applicable, maintain regular contact and open communication with families, and provide them with additional resources.

Requirements

  • High School Diploma required.
  • One year of experience in a substance use and/or mental health treatment environment required.
  • Demonstrates critical thinking, sound reasoning and judgement.
  • Effective communication skills and processes.
  • Fosters a healthy and positive team-approach with staff.
  • Demonstrated computer proficiency.

Nice-to-haves

  • Associate or Bachelors degree preferred in either social work, counseling, human services, business administration, or closely related field.
  • Experience in start-up environments is preferred.
