About the position

The Case Manager is responsible for case management and discharge planning in an acute care setting, ensuring effective communication between patients, healthcare teams, and physicians. This role involves comprehensive care planning, implementation of discharge plans, and evaluation of patient goals, while managing community resources and post-acute providers to achieve quality outcomes and patient satisfaction.

Responsibilities

  • Perform assessments/evaluations of patients in acute settings for post hospital care needs.
  • Develop a plan for post-acute care that meets the needs of the patient and their goals of care.
  • Implement the discharge plan in a timely manner to avoid delays.
  • Conduct follow-up assessments during patients' continued stay to evaluate the need for modifications to the discharge plan.
  • Perform readmission assessments on patients who readmit within thirty days of previous discharge for quality or safety concerns.
  • Handle communications with patients, families, and the medical care team to decrease conflict.
  • Promote informed decision-making and empower patients to participate in their care planning.
  • Utilize advanced knowledge to assess various statuses of patients to identify those at risk for adverse outcomes.
  • Collaborate with medical and nursing staff regarding assessment changes that influence discharge planning.

Requirements

  • Associate in Nursing or Bachelor's in Social Work (BSW); Current RN license or license with the State Board of Social Work; Or licensure with the state board of social work within six months of hire.
  • Two years of acute care experience or community resource/public health experience related to transition of care from a hospital, or completion of 300 or more internship hours at this facility prior to graduation.

Nice-to-haves

  • Bachelor of Science in Nursing (BSN) or Master's in Social Work (MSW); Current RN license or license with the state board of social work.
  • Previous hospital-based case management experience.
