About the position
The Case Manager plays a crucial role in providing outreach and support to individuals involved in the re-entry process and other justice-involved community members. This position focuses on delivering clinical and treatment support, including medication-assisted treatment and therapy, to help clients achieve their treatment goals while addressing barriers to success. The role requires both community support and case management work across various settings, including jails and outpatient clinics.
Responsibilities
- Provide service coordination and linkages to the community.
- Deliver outreach services to clients in various settings, including inpatient units, homes, and day programs.
- Assist clients in obtaining benefits, housing, and healthcare as needed.
- Provide crisis support and participate in the development and utilization of crisis prevention and safety plans.
- Encourage and facilitate the utilization of clients' natural support systems and recovery-oriented services.
- Assist in data collection and tracking for grant evaluation.
Requirements
- Bachelor's degree from an accredited educational institution or at least three years of relevant work or lived experience acceptable by MBHP waiver.
- Ability to pass certification for CPR and First Aid.
- Valid driver's license and reliable transportation for business use.
- Ability to counsel and support clients with psychiatric and/or substance use disorder diagnoses.
- Demonstrated knowledge of trauma-informed care.
Nice-to-haves
- Lived experience relative to psychiatric and/or substance use disorder preferred.
- Recovery Coaches welcome!
- Bilingual (Spanish/English) encouraged to apply.
