About the position

The Case Manager position at The Sanctuary Foster Care Services involves providing support and advocacy for children and families, ensuring their physical, emotional, educational, social, spiritual, and cultural needs are met. The role requires developing treatment plans, conducting assessments, and maintaining case files while collaborating with multidisciplinary teams and community agencies.

Responsibilities

• Develop individual treatment plans, goals, and outcome measurements within thirty days of client's admission.
• Complete service plan reviews every 90 days thereafter.
• Maintain accurate and up-to-date case files.
• Conduct monthly visits with each child and family on assigned caseload to assess needs.
• Provide crisis intervention services on a 24-hour basis, alternating weekly coverage with other Case Managers.
• Function as a liaison for children and families with DFPS and other community agencies to develop a referral and resource network.
• Attend in-service workshops and training programs and participate in agency activities and development initiatives as assigned.
• Represent The Sanctuary Foster Care Services, clients, and family members with integrity, professionalism, and honesty at all times.

Requirements

• Must have a Bachelor's degree from an accredited university in psychology, social work, health education, or a related human services field.
• Minimum of one year experience working with youth and families.
• Must be at least 21 years of age.
• Must carry a valid driver's license, current auto insurance, and have a good driving record.
• Ability to work flexible hours and effectively manage multiple assignments.
• Must comply with all state, federal, and local regulations.
