About the position

The Case Manager at American Addiction Centers is responsible for managing treatment services for a designated caseload of patients struggling with addiction and co-occurring mental health disorders. This role involves coordinating care, advocating for patients, and ensuring comprehensive treatment plans are followed. The Case Manager will work closely with various teams to facilitate recovery and provide support to patients and their families, ultimately aiming to empower individuals to achieve optimal wellness.

Responsibilities

• Manage treatment services for assigned caseload including coordination of care and discharge planning.
• Document weekly coordination of care to improve patient adherence to treatment.
• Complete aftercare assessments and discharge plans according to policy timelines.
• Facilitate life skills and recovery groups as assigned.
• Provide weekly sessions with patients addressing recovery stability and treatment adherence.
• Manage patient behavioral concerns and crisis situations as needed.
• Attend weekly treatment team meetings and communicate with other departments for patient care.
• Ensure all documentation meets policy and accreditation standards.
• Maintain confidentiality of patient information and adhere to HIPAA guidelines.

Requirements

• Bachelor's Degree in Behavioral Sciences or equivalent experience preferred.
• Minimum of 2 years' experience in addiction support roles such as counselor or social worker.
• Experience in behavioral health treatment and case management for substance use and mental health disorders.
• Knowledge of electronic health record software and documentation practices.
• Proficient in Microsoft Word, Excel, and Outlook.
• Strong communication and decision-making skills.

Nice-to-haves

• State recognized alcohol and drug counseling certification (IC&RC) preferred.
• Experience in group facilitation and community liaison roles.
