About the position

The Case Manager Supervisor will serve as a supervisor over a team of medical and non-medical case management staff under the guidance of the Program Manager. This position involves providing technical support, monitoring accuracy in the application of program regulations, and identifying areas for process improvement to support the team. The supervisor will also manage their own caseload, which may include both non-medical and medical case management, depending on the team's needs.

Responsibilities

  • Performance Management: Communicate expectations to staff, provide necessary training and continuing education, address performance issues, complete annual performance evaluations, and meet with staff consistently to monitor progress.
  • Case Management: Conduct initial assessments, provide direct social work/case management services to individuals living with HIV/AIDS, develop individualized service plans, respond to referrals, document accurately in clients' Electronic Medical Records, and assist with the referral process.

Requirements

  • Bachelor's degree in relevant discipline or equivalent work experience required.
  • Proven experience as a supervisor or in a relevant role.
  • 5 years of Case Management experience.
  • 3 years of Supervision experience or equivalent people and program management experience.
  • Detailed knowledge of Ryan White Case Management standards.
  • Ability to work independently or as part of a team.
  • Excellent written and verbal communication skills; empathy, compassion, and confidence.
  • Ability to work and communicate effectively with individuals from culturally and socioeconomically diverse backgrounds.
