About the position

The Case Manager RN - Field at CVS Health is responsible for managing a selected member population through a collaborative process that includes assessment, planning, facilitation, care coordination, evaluation, and advocacy. This role aims to meet the comprehensive health needs of individuals and families, ensuring quality and cost-effective outcomes. The position requires significant travel within Ohio and involves both telephonic and in-person interactions with members to assess and coordinate care effectively.

Responsibilities

• Serve as primary care manager for members identified for Care Management program
• Engage members to complete holistic assessments of current health status
• Collaborate with a multi-disciplinary team to develop and implement a care plan
• Educate and assist members with advanced care planning
• Monitor and evaluate effectiveness of the care plan
• Work with treating physicians to develop anticipatory symptom management plans
• Prepare members/caregivers to understand signs of functional decline
• Ensure safe and appropriate transitions of care by collaborating with discharge planners and providers

Requirements

• Minimum of 3 years of professional/clinical nursing experience
• 2+ years of care management experience in health plan, hospice, home health, or hospital setting
• Registered Nurse with an unrestricted license in state of residence
• Strong organizational, communication, and problem-solving skills
• Proficiency with electronic health records (EHR) systems
• Excellent analytic and problem-solving skills
• Ability to work independently
