About the position

Misericordia Heart of Mercy is a home to over 600 residents with mild to profound developmental disabilities. Misericordia offers a community of care that maximizes potential for persons with mild to profound developmental disabilities, many of whom are also physically challenged. We are currently hiring for Case Manager/Qualified Intellectual Disabilities Professional (QIDP) for several of our residential areas. As a Case Manager/QIDP, you will oversee the needs of the resident's programs, goals and community outings amongst many other facets of daily life. QIDPs work closely with a team of professionals to ensure quality care and programming for our residents. As a Case Manager/QIDP at Misericordia you will have the opportunity to create programs to nurture the growth of our residents- physically, mentally, emotionally, and spiritually. The QIDP creates programs where the resident's individual goals and desires are worked on in combination with skills that are need to achieve greater independence as identified by the interdisciplinary team.

Responsibilities

  • Oversee the needs of the resident's programs and goals
  • Coordinate community outings for residents
  • Create programs to nurture the growth of residents physically, mentally, emotionally, and spiritually
  • Work closely with a team of professionals to ensure quality care and programming
  • Develop rapport and build long-term relationships with residents and families
  • Manage time and resources while meeting deadlines
  • Supervise as required and work a variety of shifts/hours

Requirements

  • Minimum of 1 year experience working with persons with developmental disabilities
  • Bachelor's degree required, preferably in human services or a related field such as Psychology, Sociology, Special Education, or Rehabilitation
  • Ability to provide physical care to residents
  • Demonstrated ability to communicate effectively, orally and in writing
  • Demonstrated leadership ability
  • Ability to work well with a team
  • Commitment to ensuring the rights of residents
  • Ability to demonstrate reliability, flexibility, and adaptability

Nice-to-haves

  • Experience in developing programs for individuals with disabilities
  • Knowledge of state-funded resources for childcare support
