About The Position

People Incorporated is seeking a Case Manager II to join our Waiver Case Management team in Hennepin County. This role involves working with clients diagnosed with chronic mental illness, supporting them in connecting with resources to live as independently as possible. The position helps clients develop self-awareness for positive future planning through frequent community meetings. Case Managers develop long-term relationships with clients and operate autonomously, creating their own schedules. The position is full-time, Monday-Friday, 8am-4pm.

Requirements

  • Valid driver’s license, excellent driving record and current auto insurance
  • Bachelor’s Degree in Social Work, Special Education, Psychology, Nursing, Human Services or other fields related to the education or treatment of persons with developmental disabilities or related conditions
  • One year of experience in the education or treatment of persons with developmental disabilities or related conditions

Responsibilities

  • Perform functional assessments that lead to individual plan development and result in the client receiving access to necessary mental health and related services.
  • Conduct regular review of plan effectiveness and make adjustments as needed that result in the client receiving the maximum amount of assistance and support while monitoring the effectiveness of services received.
  • Assessing client needs and ensuring continuity of care.
  • Making ongoing referrals to resources based on assessment and current client need.
  • Maintaining necessary daily records for continuity of service and reports to funding agencies.
  • Documenting all interventions, interactions and relevant information to the treatment plan consistent with professional and organizational standards.
  • Attending client monthly meetings to monitor services.
