About the position

The Case Manager II (Street Outreach) at LifeLong Medical Care is a vital role within the primary care interdisciplinary team, focusing on providing comprehensive services to patients with complex care needs. This position involves conducting outreach, engaging with patients, assessing their psychosocial needs, and developing patient-centered care plans. The Case Manager will work in various settings, including homes, clinics, and community sites, to deliver Enhanced Case Management (ECM) and coordinate service referrals, ensuring that patients receive the necessary support for their health and social service needs.

Responsibilities

  • Conduct outreach to patients via telephone and in-person at various sites.
  • Engage with patients to assess their strengths and needs using standard intake and screening tools.
  • Involve patients and caregivers in designing and delivering services, including care plan development.
  • Facilitate referrals for internal and external resources and assist with required applications.
  • Maintain a patient caseload according to LifeLong standards.
  • Utilize data registries to manage caseload and meet program requirements.
  • Provide health education and training to patients on health management strategies.
  • Assist patients in accessing public benefits and affordable housing.
  • Communicate regularly with patients and their care team members.
  • Maintain knowledge of patients' treatment plans and facilitate service utilization.
  • Participate in team meetings to coordinate care and support patient goals.
  • Advocate for patients' needs and support their self-advocacy strategies.
  • Provide case management services for patients with complex medical or behavioral health conditions.
  • Assist with housing case management services and document readiness.
  • Provide crisis intervention and de-escalation support.
  • Document patient contacts and services in required data systems.

Requirements

  • High School diploma or GED
  • At least two years of relevant work or volunteer experience in a community-based health care or social work setting, or one year as a Case Manager I or equivalent.
  • Proficient in Microsoft Office applications (Word, Excel, Outlook) and database management.
  • Access to reliable transportation with a current license and insurance.

Nice-to-haves

  • Bachelor's Degree in Social Work, Health, or Human Services field.
  • Lived experience of homelessness, incarceration, or mental health services.
