About the position

Do you believe educating others about potential barriers to healthcare and community well-being is important? Do you have a deep passion to advocate for individuals and families to know how to access health-related resources and how to navigate challenging healthcare services and systems in San Diego? If you answered “YES”, then SAY San Diego's Neighborhood Networks program may be a great fit for you! Neighborhood Navigators are community-based Lead Care Managers (LCM) providing case management support to individuals and families undergoing chronic health conditions. Providing intensive care coordination to improve overall health outcomes addresses the broader contexts affecting health via Enhanced Care Management (ECM).

Responsibilities

  • Provide case management services through client-centered approaches such as Motivational Interviewing and strength-based interventions.
  • Manage case load of 55-60 members to include intake, client needs assessment, goal setting, develop individual service plans, advocacy for client development, and follow-up services.
  • Maintain accurate client files, document case notes in timely manner, and complete funder reports as requested.
  • Schedule and conduct home or in-person visits with clients for intake and or client servicing appointments.
  • Coordinate and conduct group facilitation may include trauma informed services, leadership development, socio-emotional curriculum, or other topics of interest.
  • Develop and maintain positive relationships with clients and other connected program partners.
  • Identify and remain knowledgeable of program related resources: self-sufficiency benefits, employment opportunities, financial literacy, healthcare and behavioral health services.
  • Contact eligible ECM members to assist in development, implementation and evaluation of Comprehensive Assessments and Social Determinants of Health Assessments, and presentation of all members during Interdisciplinary Care Team (ICT) meetings.
  • Advocate on behalf of clients with healthcare professionals, work with medical facility/hospital staff to ensure well supported care needed to maintain Medicaid eligibility.
  • Actively recruits program participants/clients using referral resources.
  • Attend and participate in partner and funder-related meetings and outreach events.

Requirements

  • Bachelor's Degree in Public Health, Child and Youth Development, Humans Services, or related field preferred.
  • Experience in case management, group facilitation, direct client service, and delivering curriculum-based interventions with youth and families.
  • Lived or Allyship experience working with complex physical, behavioral, or developmental health needs, individuals at risk of or experiencing homelessness, and/or individuals transitioning from incarceration.
  • Bi-lingual English/Spanish required.
