Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Telecare's mission is to deliver excellent and effective behavioral health services that engage individuals in recovering their health, hopes, and dreams. Telecare continues to advance cultural diversity, humility, equity, and inclusion at all levels of our organization by hiring mental health peers, BIPOC, LGBTQIA+, veterans, and all belief systems. Telecare’s community program in Central Orange County, Homeless General Population will focus on addressing the complex housing needs of up to 150 members, many of whom have long histories of homelessness and serious mental illness. The program will emphasize enhanced engagement and outreach efforts along with intensive case management, Vocational/Educational/Employment support and robust housing strategy services.
