About the position

Responsible for providing services to individuals in the case management program with severe and persistent mental illness, serious emotional disturbance, and/or intellectual/developmental disabilities. Works to assure all necessary supports and services are provided to enable the individual to achieve community inclusion and participation, productivity, and independence in home and community based settings. Responsible for advocacy, coordination, and linkage of services to provide continued care and to avoid rigidity, fragmentation, misuse and/or inaccessibility of services. Must possess the ability to link the individuals served to services and coordinate the various services to achieve successful outcomes. Provides services to assist the individuals served in gaining access to needed medical, social, education, therapeutic, and other support services. Major functions and core elements of the role includes service plan development, linking and coordination of services, assessment/follow-up and monitoring of services and advocacy.

Responsibilities

  • Provide services to individuals in the case management program with severe and persistent mental illness, serious emotional disturbance, and/or intellectual/developmental disabilities.
  • Assure all necessary supports and services are provided for community inclusion and participation, productivity, and independence.
  • Advocate, coordinate, and link services to provide continued care and avoid fragmentation or inaccessibility of services.
  • Link individuals served to necessary services and coordinate various services for successful outcomes.
  • Assist individuals in gaining access to needed medical, social, education, therapeutic, and other support services.
  • Develop service plans, link and coordinate services, assess and monitor services, and advocate for individuals.

Requirements

  • Bachelor's Degree (B.A.) in human services field
  • One year experience in the mental health field
  • Certified as a QMHP, QIDP, and/or CMHP and/or be limited or fully licensed in the State of Michigan in their relative field
  • Possession of a driver's license valid in the State of Michigan
  • Vehicle insurance as required by Michigan Law
  • Good driving record
  • U.S. Citizen or possession of necessary papers allowing holder to work legally within the United States.
