This is an outpatient community-based program serving clients (ages 18 plus) with services including housing, education, vocational support, individual and family counseling, and medication management and support services. SBHG currently employs approximately 1,900 people at more than 45 different sites throughout California. We serve more than 30,000 adults, children, youth and families annually throughout the state. Requirements

  • Associate degree plus two (2) years of experience working in a mental/behavioral health field required (Internship may be substituted for a portion of the work experience)
  • Valid California Driver’s License required and must meet Agency driver eligibility criteria.

Nice To Haves

  • Bachelor’s degree plus six (6) months of experience preferred. (Internship may be substituted for a portion of the work experience)

Responsibilities

  • Work within the program model’s framework, including appropriate Evidence-Based Practices, to provide services to clients and their families.
  • Provides services at the office, in the community, and in clients’ homes.
  • Uses a personal vehicle to drive within the community to provide services and to transport clients in the personal vehicle.
  • Provides case management services to clients and their families, as deemed necessary and appropriate through the assessment process and in collaboration with the treatment team.
  • Participates in meetings, such as Treatment Team Meetings, to ensure clients’ needs are met and recommendations are followed through.
  • Documents all services provided clearly and concisely within the Electronic Medical Record within the timelines and framework established by Stars.
  • Provides services in the community, and in client’s homes.
  • Uses personal vehicle to drive within the community to provide services and to transport client and/or client’s family members in personal vehicle.
