About the position
The Case Manager, BH position at Molina Healthcare involves working with members, providers, and multidisciplinary teams to assess and coordinate integrated care delivery, focusing on behavioral health and long-term care for high-need members. The role emphasizes ensuring quality, medically appropriate, and cost-effective care while promoting member progress towards desired health outcomes.
Responsibilities
- Completes clinical assessments of members per regulated timelines to determine eligibility for case management.
- Develops and implements a case management plan in collaboration with members, caregivers, and healthcare professionals.
- Conducts telephonic, face-to-face, or home visits as required.
- Performs ongoing monitoring of care plans to evaluate effectiveness and suggest changes as needed.
- Maintains an ongoing member case load for regular outreach and management.
- Promotes integration of services for members, including behavioral health care and long-term services.
- May implement specific wellness programs such as asthma and depression management.
- Facilitates interdisciplinary care team meetings and informal collaborations.
- Uses motivational interviewing to educate and support members during contacts.
- Assesses barriers to care and provides coordination and assistance to members.
- Collaborates with RN case managers/supervisors as needed.
Requirements
- Completion of an accredited LVN or LPN Program OR Bachelor's or Master's Degree in a relevant field.
- 1-3 years of experience in case management, disease management, or managed care settings.
- Active, unrestricted license if required for the job.
- Valid driver's license with a good driving record.
Nice-to-haves
- 3-5 years of experience in case management or related fields.
- Licensed Clinical Social Worker (LCSW), Advanced Practice Social Worker (APSW), or similar certifications.
