About the position
The Behavioral Health Case Manager at Merakey is responsible for providing consumer-driven, strength-based case management and crisis services to individuals in need. This role emphasizes a Rehabilitation/Recovery attitude and involves acting as the primary case manager for assigned consumers, ensuring their needs are met through effective coordination of services and support.
Responsibilities
- Demonstrate a Rehabilitation/Recovery attitude toward service.
- Provide consumer-driven strength-based approach to case management and crisis services.
- Act as the primary case manager for assigned consumers.
- Conduct minimal bi-weekly support contacts at the client's home or wherever most appropriate.
- Complete consumer-driven service and safety plans.
- Coordinate benefits, housing, and all other aspects of service delivery.
- Coordinate meetings with providers that also provide services for the consumer.
- Ensure Progress Notes are completed within 72 hours of service delivery and that weekly productivity paperwork is submitted per timelines.
- Ensure that paperwork for their consumers is current and not expired.
- Make attempts to locate client through family, friends, or relatives if unable to contact the client.
Requirements
- 2 years human services experience preferred.
- Valid PA Driver's license.
- A high school diploma and 5 years of mental health direct care experience in public or private human services with employment as a case management staff person prior to April 1, 1989; or
- A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years' experience in direct contact with mental health consumers; or
- A bachelor's degree with major course work in sociology, social work, psychology, gerontology, anthropology, political science, history, criminal justice, theology, nursing, counseling or education.
