About the position

The Case Management Specialist at Centinela Youth Services, Inc. is responsible for providing comprehensive strengths-based case management services to youth involved in or at risk of being involved in the juvenile justice system. This role focuses on conducting assessments, developing individualized service plans, and coordinating with various stakeholders to ensure effective service delivery. The specialist will work closely with youth and their families to support their emotional and behavioral stability, ultimately aiming to minimize their risk of involvement with the justice system.

Responsibilities

  • Conduct intake and assessment for youth and families.
  • Provide case planning and contracting, case coordination, and advocacy.
  • Conduct comprehensive evaluations of bio-psycho-social, emotional, educational, vocational, and functional needs.
  • Develop individualized service plans in coordination with community resources.
  • Support youth in achieving specified goals related to emotional and behavioral stability.
  • Provide crisis prevention, intervention, and safety planning services.
  • Maintain regular contact with service providers and referring agencies to monitor youth progress.
  • Make weekly home and school visits according to the service plan.
  • Act as a Mandated Reporter of suspected cases of child abuse and neglect.
  • Maintain up-to-date confidential records in the CYS Connect database and prepare case histories and reports.

Requirements

  • Bachelor's degree in social work, counseling, sociology, human services, psychology, or a closely related field.
  • Minimum two years of working experience with diverse youth and families or equivalent combination of education and experience.
  • Experience providing in-home and/or community-based direct services to families.
  • Basic understanding of Restorative Justice concepts and practices.
  • Moderate understanding of trauma's effects on behavior development.
  • Ability to perform initial assessments of youth and family needs and develop service plans.
  • Strong oral and written communication skills.

Nice-to-haves

  • Master's degree in social work and a minimum of 1 year of experience working with diverse youth and families.
  • Experience or knowledge of Restorative Justice.
  • Experience with Motivational Interviewing.
  • Social Justice oriented.
