About the position

The Full-time Care Navigator (MA or LPN) position at Homeward is designed for individuals who are passionate about improving health outcomes for people in rural communities, specifically throughout Todd County. The Care Navigator will play a crucial role in working closely with Homeward members and their healthcare providers to ensure that members achieve their best health. This involves conducting proactive outreach through telephonic, video, and in-person interactions to build strong relationships with members and connect them with necessary medical, behavioral, and social services, while also addressing any gaps in their care. The ideal candidate will possess a deep understanding of the local community, leveraging their expertise to advance the health of members. In this role, the Care Navigator will support the fulfillment of recommended health services, coordinate health-related activities, and mitigate any administrative or logistical barriers that may hinder access to care. Regular documentation of member encounters and engagement in continuous communication with members and their primary care providers will be essential to ensure cohesive care and successful completion of care plans. The position also involves building member health literacy and connecting members to community resources to reduce social isolation and loneliness.

Responsibilities

  • Conduct proactive telephonic, video, and in-person outreach to build relationships with members.
  • Connect members with necessary medical, behavioral, and social services.
  • Address gaps in care for Homeward members.
  • Support fulfillment of recommended health services, including obtaining prescribed medicines and coordinating scheduling of health-related activities.
  • Attend scheduled health-related appointments and testing.
  • Document all member encounters in the designated electronic platform in a timely manner.
  • Engage potential members by effectively communicating the services and value that Homeward can provide.
  • Build member health literacy and digital literacy.
  • Initiate communication with patients on completing pre-appointment requirements.
  • Conduct check-ins and/or visits with members telephonically, virtually, in-clinic, and/or in-home regularly.
  • Maintain a member panel of seniors located within specific counties and support successful completion of care plans.
  • Provide coaching to activate members in their self-care.
  • Collaborate with members' primary care providers to ensure cohesive care.
  • Identify and maintain a list of community resources and contacts to meet members' needs.

Requirements

  • Completion of a Medical Assistant program from an accredited program or school.
  • High school diploma or equivalent.
  • At least two years of experience in high-touch, patient-facing roles, preferably with seniors.
  • Passion for delivering care in rural America and ability to persuade members to take actions that support their health.
  • Expertise with the local community, geography, culture, healthcare ecosystem, and available resources.

Nice-to-haves

  • Previous experience with care navigation.
  • Community Health Worker Certification.
