About The Position

This position is assigned to a Division of Residential Services which is composed of a number of homes responsible for the provision of residential based services and support for individuals with developmental disabilities. The purpose of the division is to ensure the provision of direct and indirect habilitative care, treatment, and programming in order to promote growth, autonomy, productivity, and dignity. Services and support are provided on a 24-hour basis to individuals served as outlined in individual program plans. To ensure that the individuals served are provided a comprehensive array of training, services, and supports that enables them to achieve maximum independence in self-care, activities of daily living, including gaining and maintaining significant relationships, self-management and ensuring the protection of their rights. The Care Manager is responsible for documentation including program reviews. Development of and revision to life plans, auditing, and annual training of staff within the home.

Requirements

  • Bachelor’s degree in a human services or related field from an appropriately accredited institution, and one year of experience providing and/or developing person-centered care plans for individuals with intellectual disabilities/co-occurring disorders; or an equivalent combination of education and experience.
  • Current knowledge of the standards, regulations and licensure requirements is also required.
  • The ability to coordinate the activities of a group of professionals of varied disciplines
  • The ability to provide leadership in the development, implementation, and evaluation of treatment plans
  • The ability to develop cooperative working relationships with a variety of persons providing services to the individuals is required.

Nice To Haves

  • Supervisory experience preferred.
  • Through knowledge of concepts, practices, and techniques of the field of developmental disabilities and the population served to include evaluations, and development of comprehensive life plans.
  • Skill in writing life plans including punctuation, grammar, etc., and reports to document individual treatment plans, auditing records, and programming activities.
  • Graduation form a four-year college or university with a degree in human service field and a minimum of two years' experience in prescriptive program planning in a residential setting for the developmentally disable is also necessary.
