About the position

The RN Care Manager at Trinity Community at Beavercreek is responsible for ensuring that appropriate nursing services are provided to home health patients according to their individualized care plans. This role involves direct patient care, supervision of staff, and collaboration with healthcare teams to support patient independence and quality outcomes.

Responsibilities

  • Facilitate initial evaluation visits of patients in their homes or before institutional discharge.
  • Participate in the development of the Plan of Care, communicating with the attending physician and home care team members.
  • Provide direct nursing care based on the Plan of Care and accepted nursing practices.
  • Supervise assigned staff in the implementation of the Plan of Care.
  • Educate clients and/or families regarding new skills and activities of daily living.
  • Plan for patient needs after discharge, including equipment and supply delivery.
  • Make follow-up calls after discharge as per policies and procedures.
  • Ensure referrals to other services are communicated and followed through.
  • Participate in meetings and care conferences as needed.
  • Document homebound status, medical necessity, and nursing services provided.
  • Complete necessary documentation for billing processes within specified timeframes.
  • Comply with all home health policies, procedures, and regulations.

Requirements

  • Must be a graduate of a two-year accredited nursing diploma school or have a Bachelor's Degree in nursing.
  • Active RN license conferred by the State Board of Nursing.
  • Successful completion of a state-approved pre-licensure nursing education program.
  • Passing of National Council Licensure Examination-Registered Nurse (NCLEX-RN).
  • Must obtain and maintain mandatory state and federal requirements and certifications for practice.
  • Must be CPR certified.
  • One previous experience, preferably in a related field.
  • Proficient in using a computer and basic office equipment, including Microsoft Office.

  • Experience in home health care settings.
  • Familiarity with electronic medical records (EMR).
