About the position
The Care Manager-RN plays a crucial role in ensuring efficient patient progression through the continuum of care. This position involves collaboration with patients, families, social workers, physicians, and interdisciplinary teams to manage care coordination and discharge planning. The Care Manager-RN is responsible for enhancing patient flow, ensuring patient satisfaction, safety, and compliance with regulatory requirements while providing cost-effective care.
Responsibilities
- Collaborate with patients, families, and interdisciplinary teams to ensure efficient patient progression through the continuum of care.
- Manage optimal care coordination and discharge planning to enhance patient flow and continuity of care.
- Monitor the provision of cost-effective care in the most clinically appropriate setting.
- Facilitate compliance with regulatory requirements and organizational performance standards.
- Conduct comprehensive assessments and evaluations of individual patient needs.
Requirements
- Associate's Degree or Diploma in Nursing is required.
- License as a Registered Nurse (RN) in the state of practice is required.
- 2 years of clinical experience is required.
- Basic Life Support Certification (BLS) is required within 60 days of hire.
Nice-to-haves
- Bachelor's Degree in Nursing is preferred.
- Prior case management experience is preferred.
- Quality/Process Improvement experience is preferred.
- Case Management certification (ACM, CCM, RN-BC) is preferred.
