About the position

The Care Manager at Alpert Jewish Family Service plays a crucial role in the Guardianship Program, providing comprehensive support and services to vulnerable clients. This position involves coordinating intake, evaluation, and implementation of services, as well as maintaining detailed records and documentation. The Care Manager works closely with clients, their families, and various stakeholders to ensure the well-being and independence of those under guardianship, while also participating in legal processes and interdisciplinary planning.

Responsibilities

• Coordinate intake, evaluation, implementation, and provision of services for clients.
• Conduct bio-psychosocial assessments in clients' home settings.
• Develop service plans based on assessments to meet clients' needs.
• Act as the first-line contact with vendors and agencies involved with clients.
• Document all activities and maintain case records to meet quality standards.
• Attend court sessions and participate in Guardianship hearings as required.
• Communicate with wards, family members, and professionals to gather information and provide support.
• Prepare and submit annual reports for the renewal of guardianship.

Requirements

• Bachelor's degree in Social Services or related field.
• 3-5 years of experience in a similar role.
• Experience with geriatric and special populations.
• Knowledge of MS Office (Word, Excel) and familiarity with database systems.
• Valid Florida driver's license with a good driving record.
• Strong communication and interpersonal skills.
