About the position

As a Care Manager at Care Design New York, you will play a crucial role in supporting individuals with intellectual and/or developmental disabilities by helping them identify and achieve their personal goals through the development and management of person-centered Life Plans. This position requires strong relational and organizational skills, as you will be working in a hybrid environment that includes home visits, office meetings, and remote work.

Responsibilities

• Advocate for members to ensure informed decision making and appropriate guardianship.
• Schedule and lead meetings with members and their interdisciplinary teams to develop comprehensive Life Plans.
• Implement, update, and monitor Life Plans, facilitating reviews and approval processes.
• Ensure integration of necessary supports and services, including medical, behavioral, and community-based services.
• Submit all required documentation in a timely manner, including Life Plans and progress notes.
• Assist members in maintaining benefits such as Social Security, Medicaid, and Food Stamps.
• Help members resolve living problems related to housing, utilities, and safety.
• Report any observed or reported abuse or neglect immediately.

Requirements

• A Bachelor's degree with 2+ years of relevant experience, OR
• A License as a Registered Nurse with 2+ years of relevant experience, OR
• A Master's degree with 1+ year of relevant experience.
• Strong communication skills, both verbal and written.
• Excellent organizational and interpersonal skills.
• Bilingual in English and Cantonese.
