The Care Coordinator (CCBHC) is responsible for providing coordination of services for patients receiving services. This role involves identifying issues related to employment, education, healthcare, social connectedness, and criminal justice, and coordinating outpatient services. The Care Coordinator will maintain regular communication with referral sources and primary care providers, participate in staffing for continuity of care, and make or assist with outside referrals. They will also assist in scheduling treatment, arranging transportation, and continuously developing individualized treatment plans. The role requires representing the agency professionally, ensuring or assisting with referrals for medical issues, linking families with community services, advocating on behalf of families, and assisting with transitional planning. Chart deficiencies must be corrected promptly, and all services documented in accordance with KDHE/JJA Standards. Crisis intervention and adherence to weekly direct service expectations are also key components. The position requires obtaining 32 hours of training every 2 years for professional development and demonstrating interest in the company's long-term and short-term goals. Other duties may be assigned by supervisors.
