About the position

The Care Coordinator (Certified Peer Specialist - CPS) position at View Point Health involves providing intensive case management services to individuals with severe and persistent behavioral health disorders. The role focuses on preventing hospitalization and supporting clients in community settings, utilizing personal recovery experiences to aid in their recovery goals. The Care Coordinator works as part of a multidisciplinary team to deliver both direct and indirect clinical services, ensuring clients receive the necessary support and resources to manage their mental health effectively.

Responsibilities

  • Provides intensive case management services for consumers with severe and persistent behavioral health disorders.
  • Conducts face-to-face comprehensive assessments to identify problems and service needs.
  • Spends 85% or more of total scheduled work hours in the community using personal vehicles to meet with clients.
  • Utilizes personal recovery experience to support consumers in meeting daily recovery goals and during crisis episodes.
  • Functions as part of an interdisciplinary team to provide clinical and supportive services.
  • Writes timely progress notes that relate directly to treatment outcomes described in the service plan.
  • Monitors consumer progress and compliance with the service plan.
  • Identifies unmet needs and modifies the treatment plan as indicated.
  • Establishes linkages and coordination of services with community agencies.
  • Provides symptom management, behavior management planning, and crisis intervention services.

Requirements

  • Bachelor's degree in a Social Services related field from an accredited college or university AND one year of experience in a social service-related field.
  • High school diploma or GED AND three years of experience in a social services related position.
  • Certified Peer Specialist (CPS) Certification or eligibility to obtain based on personal lived experience.
  • Valid Georgia driver's license and Motor Vehicle Record in accordance with company policy.
