About the position

The Care Coordinator at Behavioral Health Partners of MetroWest (BHPMW) plays a crucial role in providing collaborative care coordination and wellness support to individuals with behavioral health and substance use needs. This position focuses on engaging Enrollees in their care, conducting assessments, developing treatment plans, and facilitating communication among care teams to ensure comprehensive support and services are provided.

Responsibilities

  • Utilize effective engagement strategies to ensure Enrollees lead in their BHCP services.
  • Conduct outreach and engagement activities with assigned Enrollees to enroll them in the BHCP program.
  • Complete comprehensive assessments collaboratively with engaged Enrollees and conduct annual re-assessments.
  • Develop person-centered treatment plans with each engaged Enrollee and update them as required.
  • Create advanced directives, acute care plans, and crisis plans with engaged Enrollees as needed.
  • Assemble Care Teams for engaged Enrollees and facilitate communication and coordination.
  • Support Enrollees during care transitions, including discharge planning and post-discharge meetings.
  • Assist the RN with medication reconciliation functions as required.
  • Provide health and wellness coaching to engaged Enrollees and connect them to community supports.
  • Collaborate with providers, Care Team members, and stakeholders to deliver CP supports according to treatment plans.
  • Meet expectations related to supporting the required number of Enrollees.
  • Maintain effective communication with staff, Enrollees, and stakeholders, promoting strong relationships and customer service.
  • Complete required trainings and participate in BHCP team meetings.
  • Identify community resources and develop natural supports for clients.
  • Consult with Clinical Care Managers and other CP Team members as needed.

Requirements

  • Bachelor's Degree in a field related to human services with a minimum of 1 year of experience, or High School diploma/GED with a minimum of 3 years of experience working with adults in community-based or medical settings.
  • Ability to maintain personal and professional boundaries.
  • Strong communication, follow-through, collaboration, and customer service skills.
  • Proficiency in contemporary Windows operating systems and Office suites, especially Word and Excel.
  • Excellent organizational, time management, and problem-solving skills.
  • Ability to address issues of substance use and mental illness openly.
  • Excellent written and verbal communication skills.
  • Valid driver's license and access to an operational and insured vehicle.
