The Emergency Room RN Care Coordinator works with neonate, adolescent, adult, and geriatric patients, families, medical staff, and community agencies to determine the appropriate setting of care and appropriateness of hospitalization, as well as to assess post-hospital care needs. This role provides the necessary interventions in order to assure effective and efficient clinical resource management. The RN Care Coordinator also provides intervention regarding adjustment to hospitalization, crisis intervention, and psychosocial assessment.

Responsibilities

  • Work with patients, families, medical staff, and community agencies to determine appropriate care settings.
  • Assess the appropriateness of hospitalization and post-hospital care needs.
  • Provide necessary interventions for effective clinical resource management.
  • Assist with adjustment to hospitalization and provide crisis intervention.
  • Conduct psychosocial assessments.

Requirements

  • Graduate of an accredited Registered Nursing School (RN) with a current GARN license.
  • One (1) year previous experience in case management, utilization review, or social services.
  • Ability to interact with patients, families, physicians, and hospital staff.
  • Excellent oral and written communication skills.
  • Ability to type accurately at an approximate rate of 40-45 wpm.

Nice-to-haves

  • BSN
  • Three (3) years' experience in case management, social services, or related field.
  • Certification as Certified Case Manager.
