About the position

Sage Health is dedicated to providing concierge primary care and wellness to seniors without the associated fees. We build accessible neighborhood health centers that cater to all healthcare needs of our patients, partnering with Medicare Advantage plans that cover primary care. Our physicians manage a patient panel of 400 or fewer, collaborating with top specialists and hospitals to ensure optimal health outcomes. Our focus is on maintaining the health of our seniors through a value-based care model, which emphasizes quality over quantity in patient care. We are looking for a Cardiologist to join our team, which includes physicians, advanced practice professionals, and support staff, to enhance the care experience for our patients.

Responsibilities

  • Evaluates and treats center patients in accordance with standards of care.
  • Assists in the coordination of patient services, including specialty referrals to cardiology subspecialists and vascular surgeons.
  • Acts as an active participant and key source of medical expertise within the care team.
  • Partners with primary care physicians and nurse practitioners in a cooperative practice environment.
  • Completes all medical record documentation daily, working with a quality-based coder.
  • Follows policies and protocols defined by Clinical Leadership.
  • Meets with primary care physicians and market medical director to discuss quality of care and review outcome data.
  • Focuses on direct patient care, including patient-facing time and administrative tasks related to care.
  • Supports longitudinal care model for heart failure, focusing on disease management and prevention.
  • Supports readmission reduction initiatives for heart failure and CAD.
  • Creates a framework for upstream stroke prevention.
  • Coordinates care for CKD stage 5 patients needing AV graft placement prior to hemodialysis.
  • Interprets and documents test results related to cardiovascular function.
  • Coordinates patient care with cardiology subspecialists.

Requirements

  • Graduate of accredited MD or DO program of accredited university.
  • Current and unrestricted medical license or willingness to obtain a medical license in the state of practice.
  • Board Certification in Cardiology.
  • Active and unrestricted DEA license.
  • Medicare Provider Number.
  • Medicaid Provider Number.
  • High level of skill in interpersonal relationships and communication with colleagues and patients.
  • Engagement in a team-based care model.
  • Willingness to learn and adapt to a value-based care setting.
  • Superior patient/customer service skills.
  • Basic computer skills, including email and EMR.
  • Required to be screened for TB.

Nice-to-haves

  • Knowledge of HEDIS/STARS metrics.
  • Knowledge of CAHPS and HOS.
  • Experience working in medically underserved or culturally diverse communities.
  • Experience with electronic health records.
  • Experience in Medicare Advantage Global Risk Models.
