About the position

The Bilingual Wraparound Care Coordinator at the Latino Community Development Agency (LCDA) is responsible for providing targeted support services to families, ensuring a child-centered approach that addresses the needs of the whole family. This role emphasizes the strengths of the family and considers the child's age and cultural heritage. The Care Coordinator will work closely with families in their homes and communities, facilitating wraparound services and meetings, while also monitoring and evaluating the services provided.

Responsibilities

• Meet with families in their homes to provide wraparound services at convenient hours.
• Schedule, coordinate, and facilitate Wraparound/family team meetings.
• Complete initial wrap plans, strengths, culture discoveries, and intakes as needed.
• Update and modify wrap plans as necessary.
• Monitor and evaluate services provided to families.
• Provide immediate crisis intervention on a 24-hour basis.
• Maintain close communication with the Project Director, FSP, and other clinical and administrative staff.

Requirements

• High School diploma or equivalent with 36 months of Behavioral Health experience, or
• 60 college credit hours in any field with 12 months of Behavioral Health experience, or
• Bachelor's or master's degree in any field with 6 months of Behavioral Health experience.
• Ability to effectively communicate verbally with Spanish/English speaking consumers.
• Computer literate.

Nice-to-haves

• Specialized training and/or certification in mental health services or a related field, such as Juvenile Case Management Certification or SOC training.
