About the position

Seton Home works to break the cycle of abuse and poverty by providing a caring home, education, and support services necessary to transform the lives of pregnant and parenting teen mothers and their children. The Case Manager is responsible for maintaining a caseload where the work requires finding foster family and Kin, foster placements or other opportunities to take custody of children who have entered the country without legal approval. This role will serve as a child advocate within a culturally competent perspective and requires the ability to work rapidly in a changing environment. The Case Manager will oversee and coordinate all case management services to ensure client needs are met.

Responsibilities

  • Coordinate all referrals, service planning, and client documentation for assigned caseload.
  • Serve as the liaison with stakeholders including legal providers and Immigration court.
  • Focus on safe and efficient family reunification.
  • Conduct intake interviews and assess clients.
  • Review, complete and submit all required documentation with the family reunification packet.
  • Coordinate case management services for UAC children including coordination and preparation of each child's assessments, individual service plans, family reunification packets and discharge plans.
  • Screen for human trafficking concerns.
  • Participate in weekly supervision with Lead Case Manager.
  • Participate in workshops, seminars, education programs, and other activities that promote professional growth and development.
  • Ensure timely reporting and documentation of serious incidents in compliance with TDFPS Minimum Standards, Office of Refugee Resettlement-Division of Unaccompanied Children's Services National Care Standards, and other stakeholders.
  • Maintain a full caseload, complete with ongoing individual service plans.
  • Maintain regular contact with clients.
  • Other duties as assigned by Director, Senior Director or Vice President.

Requirements

  • Bachelor's degree in behavioral sciences, human services or social service fields or business administration or management.
  • Preferred 2 years' experience of progressive employment experience within a social service or child-care agency.
  • Reliable transportation.
  • Valid driver license.
  • Valid vehicle insurance.
  • Submit to a state and federal background check, Child abuse and neglect registry check, and Tuberculosis screening.
  • Be physically, mentally, and emotionally capable of performing assigned tasks and have the skills necessary to perform essential duties and responsibilities.
  • Able to show proof of immunity to vaccine-preventable diseases transmitted by the respiratory route if working directly with unaccompanied children.
  • Working knowledge of TDFPS Minimum Standards, and Office of Refugee Resettlement- Division of Unaccompanied Children's Services National Care Standards.
  • Experience with computer software and MS Office (i.e. MS Word, Excel, Outlook, and OneDrive).
  • Must be detail oriented, organized, self-motivated, work well independently and on a team.
  • Must have good written and verbal skills.
  • Must have good critical thinking and problem-solving skills.
  • Bilingual: Fluent in English/Spanish with regards to reading, writing, speaking, and comprehension of the spoken language.
