About the position
This position provides comprehensive care coordination for members in the Behavioral Health department at Banner Del Webb Medical Center. The role involves assessing member needs, facilitating provider engagement, and ensuring quality clinical services are delivered effectively. The position is accountable for managing individual members across the healthcare continuum to achieve optimal outcomes in clinical care, financial performance, and patient satisfaction.
Responsibilities
- Manages individual members/patients across the health care continuum to achieve optimal clinical, financial, operational, and satisfaction outcomes.
- Acts in a leadership function with process improvement activities for populations of patients/members.
- Evaluates the appropriateness of care, optimizing patient outcomes.
- Establishes and promotes collaborative relationships with providers and other members of the health care team.
- Communicates pertinent information to fulfill utilization and regulatory requirements.
- Develops and manages the interdisciplinary member discharge plan, effectively communicating it across the continuum of care.
- Educates internal members of the health care team on care management and managed care concepts.
Requirements
- Bachelor's degree in social work or related field or equivalent experience.
- Five years of experience in care management or related field.
- Working knowledge of care management, hospital, and community resources.
- Demonstrated critical thinking skills and problem-solving abilities.
- Effective communication and time management skills.
- Ability to work effectively in an interdisciplinary team format.
- Arizona Fingerprint Clearance Card at the time of hire.
