About The Position

Sheppard Pratt is searching for a Registered Nurse to serve as Assistant
Director for its Health Home program split between Gaithersburg and Lanham, MD.

What to Expect as an Assistant Director

You will coordinate somatic healthcare in tandem with mental illness, emotional
disturbance and/or substance use disorder treatment. You will identify the gaps
and barriers between behavioral and physical healthcare to develop wellness and
prevention initiatives that ensure all care needs are met.

Once assigned a patient, you will ensure the implementation of their treatment
plan in partnership with program leadership and staff, the individual, and their

interpersonal skills and a desire to develop connections with mentally ill

Requirements

• Must be a Registered Nurse in Maryland at time of application.
• Must have 1-3 years of nursing experience.
• Must have car insurance and access to a vehicle.

Responsibilities

• Monitor treatment plan goals.
• Coordinate access to medical providers and hospitals, including admission and discharge.
• Ensure patients complete all required assessments, screenings, and immunizations.
• Keep abreast of current trends and developments in the field.
• Participate in the evaluation of program effectiveness and ensure continuous improvements in the quality of services.
• Assist in training of Health Home staff, specifically nurses.
• Identify quality improvement opportunities and population-level care management.
• Develop and maintain partnership with external entities (hospitals, medical providers, and community organizations) that play a role in treatment plans.
