About The Position

The Assessor Coordinator reports directly to the Home Care Manager. The Assessor Coordinator is responsible for assessment/reassessment services with ongoing client care planning and management. The Assessor Coordinator is accountable for establishing and maintaining positive liaison between the Health Centre and Home-Based Services. The Assessor Coordinator is accountable for screening, referring, reviewing, planning and facilitating safe care for individuals.

Requirements

• Registered/eligible for registration with the appropriate licensing body
• Ability to travel when required
• Bachelor of Science in Nursing (BScN)
• Bachelor of Psychiatric Nursing
• Bachelor of Physical Therapy
• Psychiatric Nursing diploma
• Bachelor of Occupational Therapy
• Bachelor of Social Work
• Nursing diploma
• Practical Nursing diploma with 4 years experience
• Bachelor of Psychology
• Basic - Organizational skills
• Negotiation skills
• Decision making skills
• Problem solving skills
• Intermediate - Communication skills
• Computer skills
• Basic - Assessment skills
• Ability to prioritize and engage in empowerment dynamics with family & others.
• Knowledge of community resources
• Ability to refer to, network, and work cooperatively with community agencies
• Knowledge in ethnic and cultural diversity
• Strong interpersonal skills
• Ability to work as a member of a multi-disciplinary team
