About The Position

Reporting to the Manager, the Assessor Coordinator - Degree is responsible for the coordination and evaluation of LTC screenings, and client supportive case management. The Assessor Coordinator degree will ensure that the case management services are consistent with the policies, vision, mission and philosophy of the SHA. The position will coordinate the utilization of the electronic assessment system RAI-MDS (hereafter referred to as MDS) as the screening tool for continuing care eligibility and placement and alternative level of care services placement within the SHA. This position will manage the placement of continuing care clients into the appropriate vacancy, providing an effective program with continuous quality improvement and management practices to ensure efficient utilization of the system consultation with Directors of assigned Integrated Service Areas.

Requirements

  • Bachelor Degree in a related Health Science or Social Science discipline
  • Registered/eligible registration with appropriate licensing body if applicable
  • Basic computer skills including Microsoft Office programs (Word/Excel/Outlook)
  • Ability and flexibility in adapting to changing situations and environments
  • Ability to communicate effectively within the virtual team
  • Work with individuals' family, caregivers, and community organization supports
  • Ability to adapt to change or unexpected developments
  • Ability to take initiative

  • Registered/eligible with the Saskatchewan Association of Social Workers (SASW)
  • Valid driver's license, where required
  • Ability to provide a valid Criminal Record Check (CRC) dated within six (6) months and including a vulnerable sector search for interview
