About the position

The position is for a clinical pharmacist who provides direct face-to-face care within a progressive clinical pharmacy practice. The pharmacist will manage a chronic disease patient panel focusing on managed care and high-risk patients with uncontrolled chronic disease states such as diabetes, hypertension, heart failure, hyperlipidemia, obesity, and anticoagulation. The role involves optimizing medication therapy through a collaborative scope of practice.

Responsibilities

  • Demonstrates expertise in clinical practice and manages a panel of patients with chronic diseases.
  • Assesses all medications and develops individualized care plans to ensure appropriate drug therapy.
  • Provides clinical support for complex cases and acts as a resource for others.
  • Leads and participates in medication safety advancements and performance improvement activities.
  • Participates in specialty work groups, councils, committees, and ad hoc teams.
  • Serves as a clinical pharmacy resource to ensure cost-effective medication therapies.
  • Collaborates with other patient care practitioners to deliver optimal drug therapy.
  • Supports clinical outcomes including improved quality measures and patient satisfaction.
  • Leads training and educational discussions regarding ambulatory care pharmacology.
  • Serves as a preceptor for interns, students, and residents.
  • Contributes to literature through publications, research, and presentations.

Requirements

  • Pharmacist license issued in the state(s) of practice.
  • Specialty certification required to be obtained within one year of hire.
  • Bachelor’s Degree in Pharmacy or PharmD.
  • PGY1 ASHP residency required.
  • Typically requires 5 years of experience or relevant specialized PGY2 residency.
