About the position

The position is for a clinical pharmacist who will provide direct face-to-face care within a progressive clinical pharmacy practice. The pharmacist will manage a chronic disease patient panel with a focus on managed care and high-risk patients with cardiology-related chronic disease states, including heart failure (HF), hypertension (HTN), hyperlipidemia (HLP), obesity/diabetes (cardiometabolic), and anticoagulation. The role involves optimizing medication therapy through a collaborative scope of practice.

Responsibilities

  • Demonstrates expertise in clinical practice and manages a panel of patients with chronic disease using comprehensive medication management services.
  • Provides clinical support for complex cases and acts as a resource for others.
  • Leads and participates in medication safety advancements and performance improvement activities.
  • Participates in specialty work groups, councils, committees, and ad hoc teams.
  • Serves as a clinical pharmacy resource to ensure clinically appropriate cost-effective medication therapies.
  • Collaborates with other patient care practitioners to deliver optimal drug therapy.
  • Supports clinical outcomes including improved quality measures and decreased hospital admissions.
  • Leads training and educational discussions regarding ambulatory care pharmacology and therapeutics.
  • Serves as a preceptor for interns, students, and/or residents.
  • Contributes to literature through publications, research, and presentations.

Requirements

  • PGY1 Residency (PGY2 Cardiology strongly preferred or PGY2 Ambulatory Care with substantial cardiology experience post-residency preferred).
  • Board Certification (current / within year).
  • Pharmacist license issued in the state(s) of practice.
  • Specialty certification required to be obtained within one year of hire.
  • Bachelor’s Degree in Pharmacy or PharmD.
  • Typically requires 5 years of experience or relevant specialized PGY2 residency.
