About the position
The Rehabilitation Advanced Practice Provider (NP/PA) at St. Charles Medical Group is responsible for delivering high-quality and compassionate care to rehabilitation patients. This role involves managing the physical and psychosocial care needs of patients with various diagnoses, including stroke and spinal cord injuries. The provider will collaborate with physicians and multidisciplinary teams, perform patient assessments, coordinate care, and ensure compliance with regulations. The position requires active patient contact, documentation, and participation in the overall care process to facilitate successful patient transitions to the community.
Responsibilities
- Perform patient examinations and record medical history.
- Order and interpret laboratory and diagnostic procedures.
- Determine diagnosis and therapeutic plans based on data integration.
- Administer and prescribe medications in compliance with state law.
- Order and perform medical and surgical procedures as per guidelines.
- Educate patients and families about medications and treatments.
- Collaborate with healthcare professionals to manage patient health needs.
- Maintain and review patient records and documentation accurately.
- Triage patient calls and provide consultations as needed.
- Support departmental goals and promote a healing environment.
Requirements
- Successful completion of an approved Physician Assistant or Nurse Practitioner program.
- Unrestricted licensure in Oregon as a Physician Assistant or Nurse Practitioner.
- NCCPA certification for Physician Assistants or AANC, NCC, or AANP certification for Nurse Practitioners.
- Unrestricted Federal DEA number and ability to participate in Medicare/Medicaid.
- Current BLS certification; ACLS certification if practicing sedation.
- Experience using EMR software.
